I am a new patient, what forms do I need to fill out?
Our New Patient form can be found here. Please print it out, complete it and bring to your appointment.
How do I find your office?
We are located at 2310 Mountain View Blvd. Once you arrive, head to the elevator and go to Level B. Our office is the first door on the right!
Are you accepting New Patients?
We are! Give us a call to schedule your appointment now – 541-884-1331.
What insurance do you accept?
We accept almost all insurances except for OHP and Delta PP Plans. We accept Atrio or Medicare if you are currently enrolled in a dental plan. If you are unsure if you’re covered, please call your insurance company directly.
Do you have financing options?
Yes! While we don’t have payment plans, we do offer Care Credit.
Do you make crowns?
We do! We make the crowns in our office so you can leave with them the same day as your appointment.
Do you offer bleach treatments?
We offer a few options to get the bright white teeth you’re looking for. We do Bleach Whitening in office or we have trays you can do from home. Overall, the at-home gel lasts longer and is more cost-effective.
Can I eat or drink after a filling?
Yes, unless you received an amalgam/silver filling. If that’s the case, you’ll want to avoid hard foods for 24 hours.
I have been going to Dr. Smith for all my dental needs for years. The staff is great, very accommodating and will do anything to squeeze me in when I need an emergency appointment. Dr. Smith and the Dental Hygienists always treat me like a friend rather than a patient. They ask about work, family, and my daughter. I have never had a bad experience with Dr. Smith, I never have to wait, I know that I will get in at my scheduled time and be out shortly there after. Last year I needed to have a wisdom tooth pulled, my appointment was at 4:00 I sat down in the chair and was walking out of the office by 4:16. They are reasonably priced compared to many other dentists in town. But I would choose Dr. Smith even if he was the most expensive! Thanks for all you do for your patients Andrew!
